Our Geriatric Care Managers will conduct a comprehensive safety assessment to determine the fall risk of your loved one, then design a fall prevention program to help maintain or improve mobility, independence and navigation skills. The plan includes recommendations for adding safety and adaptive equipment where needed. Many risk factors for falls can be eliminated, including: taking four or more medications, foot problems or unsafe footwear, a drop in blood pressure upon standing, vision problems, and tripping hazards in the home. When a fall prevention program is followed, the risk of falling can be dramatically reduced. Alzheimer's & Dementia Care

Our Geriatric Care Managers will conduct an in-depth assessment, interview the designated medical team and confer with family members to design a care plan that promotes wellness and safety. Our caregivers receive special training for working with clients who have Parkinson's, so they understand the unique challenges such as unanticipated stiffening or rigidity that can cause other motion challenges and risk of falling. Our caregivers also know to anticipate a weakening voice and bouts of depression. End-of-Life Care
