Rehabilitation Management

Rehabilitation can be an important part of the recovery process for elders who have suffered an illness, injury, or operation. It can take place in an inpatient setting following an "event" such as a stroke, hip fracture, or joint replacement. Rehabilitation can also occur in an outpatient clinic or even in the individual's home. Regardless of where it takes place, rehabilitation is an ongoing process that bridges the gap between the elder's abilities and the demands of everyday living, and may only involve one type of therapy two or three times per week.

Outpatient or home therapy has the same goal as inpatient therapy-to help an individual become as independent as possible-but this goal may be achieved over a longer period of time than in inpatient rehabilitation. Often, outpatient or home therapy is recommended after a patient is discharged from an inpatient rehabilitation program to help make the transition from the institution to home safe.

Fedelta Geriatric Care Managers specifies rehabilitation as part of a patient's care plan and works together with physical therapists, occupational therapists, recreational therapists, speech therapists, audiologists, social workers, and psychiatric nurses as required to see that the patient's needs are met.

Our Caregivers are trained to identify and report symptoms to our professional staff members. The caregivers adopt a psychological approach in treating patients. The goal of every rehabilitation program is to help the affected person lead a normal and active life. Traditional rehabilitation strategies combined with social contacts can help increase the involvement of patients in these programs.
