At Fedelta Home Care, we assign each client a Care Team to ensure his or her needs are met with integrated, coordinated and compassionate care. Our dedicated team of home health care professionals includes:

• A Care Coordinator is the primary day-to-day contact and central communications point for both the family and their loved one.
• A Social Worker is the pivotal link between Fedelta Home Care, the in-home caregiver and client.
• A Registered Nurse develops, modifies and oversees the customized care plan and supervises the caregiver for your loved one. Each individual care plan outlines the appropriate combination of elder care personnel and home health care services that will help keep your loved one happy and safe.
• A Caregiver is screened and carefully selected to address the individual needs of the client, whether he or she is living at home or in another residential facility. The caregiver may be a Licensed Caregiver, Certified Home Health Aide, Certified Nursing Assistant or in-home companion.
