Fedelta's 4 Steps to Better Home Health Care - Getting Started
Take the first step to better care…

Fedelta Home Care is here so seniors can live independently without compromising their
health or safety. Whether you need a live-in caregiver, or someone to visit for a couple hours a day, Fedelta Home Care can help you. No service is too small, or too large.


Step One - Contact Fedelta Home Care for an Initial Consultation

During this initial conversation, we can answer your questions about Home Care Services and briefly assess your needs.   Following this brief assessment, we can identify a Care Manager for you and your family.  We would then move to Step Two.



Step Two – Conduct Full Assessment

Your Fedelta Care Manager will meet with you for a free in-home consultation or a meeting at the Fedelta Home Care office.   During this initial meeting with you and your family, the Care Manager assesses the client’s individual needs and records physical, lifestyle and personal preferences.  We might ask questions such as:

  • Are you taking any medications?
  • How are those medications managed?
  • What do you like for breakfast?
  • Do you prefer a bath or a shower?

Based on the information gathered during this assessment we will provide you an your family with your pricing options.



Step Three – Develop Detailed Care Plan

Your Fedelta Care Manager prepares and reviews a comprehensive home care service plan which outlines specific instructions for: 

  • Personal care: bathing, eating, dressing, toileting
  • Household care: cooking, cleaning, laundry, shopping 
  • Health care: medication management, physicians’ appointments
  • Emotional care: companionship, meaningful activities, conversation 

Step Four – Select Caregiver & Begin Orientation

Once you agree to the recommendations in the care plan, we recommend the Fedelta Home Care caregiver who has the most appropriate experience, skills and personality for your situation.  You and your family meet the caregiver, and if you feel comfortable, the home care service begins.

Fedelta Home Care provides an orientation for the caregiver, where we communicate all the information supplied during the consultation.



Step Five – Ongoing Quality Control and Care Revisions

Your Fedelta Care Manager contacts you and your family routinely to discuss your caregiver services.   Additionally, our Supervisors will perform periodic visits to the home to assess your caregiver, your level of care and determine if changes are needed.
