Our People

• A Care Coordinator is the primary day-to-day contact and central communications point for both the family and their loved one.
• A Social Worker is the pivotal link between Fedelta Home Care, the in-home caregiver and client.
• A Registered Nurse develops, modifies and oversees the customized care plan and supervises the caregiver for your loved one. Each individual care plan outlines the appropriate combination of home care personnel and home health care services that will help keep your loved one happy and safe.
• A Caregiver is screened and carefully selected to address the individual needs of the client, whether he or she is living at home or in another residential facility. The caregiver may be a Certified Home Health Aide, Certified Nursing Assistant or in-home companion.

Fedelta Home Care employs a Quality Assurance Review Board to evaluate feedback, recommend specific improvements and address anticipated challenges as follows:

• Proactively solicits feedback from clients and their families, our referral partners, and our caregivers. We work to resolve any issues and suggestions immediately.
• Encourages feedback from our caregivers, who have front-line observations on ways to improve the service for our clients.
• Conducts scheduled and unscheduled onsite visits to ensure that our caregivers are meeting our standards and your expectations.
