Geriatric Care

A Geriatric Care Manager (GCM) is an RN Care Manager who has additional education and expertise in senior care issues. Fedelta's GCM's provide all of our Care Management services, with a focus on the specialized needs of seniors.

Because of their broad experiences and familiarity with community resources for seniors, Geriatric Care Managers often suggest solutions and resources the client had not considered. They also help seniors and their families navigate the complex world of insurance and benefits, to make sure they receive everything they are entitled to.

In addition to helping seniors and their families directly, GCM's can act as your informed connection with a range of other professionals who are part of your elder care network, including the following service providers:

Attorneys or trust officers.
A GCM can serve as both elder advocate and intermediary with financial and legal advisors, and is a good source of referrals for families who need services from these professionals.

Physicians and other healthcare providers.
The GCM is an ideal liaison between doctors and other health professionals, and the elder patient and family members. It's very common for our GCM's to meet clients and their families for the first time at the hospital or a skilled nursing home.

Social workers and discharge planners.
Hospital and nursing home social workers and discharge planners want to know that their senior patients have someone to coordinate their care and assist them on a long-term basis. They trust our Geriatric Care Managers to do so.

Senior living communities.
