Ongoing RN assessment ensures that necessary changes in medication, food plan or diabetes monitoring are made promptly, and caregiver assistance level is altered when needed to maintain your highest level of safety, independence and health while continuing to enjoy your established routine in the comfort of your home.

Assistance with regular blood sugar monitoring as prescribed by your physician.
Assistance with planning, shopping and preparing meals and snacks according to your food guidelines.
Encouragement and supervision of appropriate physical activity/exercise.
Ongoing diabetes education.
Assessment of skin and feet.
Ongoing assessment/monitoring of other contributing health conditions.
Communication with medical care team to ensure prompt response to changes in condition to prevent ER visit or hospitalization.
