Please allow our specially trained in-home care team of caregivers and nurses to assist you in management of your diabetes. With diet and medication compliance, glucose monitoring and a personalized physical exercise program you can continue living in your home, enjoying the comfort of familiar surroundings, maintaining your daily routine and avoiding hospitalizations and ER visits.
Following a comprehensive assessment completed by a Fedelta Care Manager, you will be assigned specially trained caregivers who are supervised and supported by the Care Manager who will also provide nursing care as needed.

Care provided is focused on your individual needs with attention to maintaining blood glucose levels, providing meals consistent with dietary requirements, encouragement and supervision of prescribed physical exercise plan and providing ongoing education and lifestyle modifications. Ongoing assessment
ensures that necessary changes in medication, food plan or diabetes monitoring are made promptly, and caregiver assistance level is altered when needed to maintain your highest level of safety, independence and health while continuing to enjoy your established routine in the comfort of your home.
Specialized care addressing needs of people living with diabetes includes:
Assistance with regular blood sugar monitoring as prescribed by your physician.
Assistance with planning, shopping and preparing meals and snacks according to your food guidelines.
Encouragement and supervision of appropriate physical activity/exercise.
Ongoing diabetes education.
Assessment of skin and feet.
Ongoing assessment/monitoring of other contributing health conditions.
Communication with medical care team to ensure prompt response to changes in condition to prevent ER visit or hospitalization.
Accompaniment client to medical appointments.
Assistance with personal care and grooming.
